The Collaborative is a community of agencies working together to advanced value-based care and social services innovation among more than forty community agencies spanning every region of Ohio.
Current Initiatives
Clinically Integrated Health Care Network (CIN)
With a focus on children and families, our clinically integrated network encompasses provider agencies in every region of the state, 3,500+ health care practitioners, and collectively serves more than 60,000+ clients each year. Together, we are enhancing our behavioral healthcare performance, quality, and efficiency to improve outcomes for kids and families.
Bridges Program
Ohio’s program which extends foster care to eligible young adults by equipping those who age out of the system with secure housing, employment, and pursue of higher education. Help Spread the Bridges Mission by utilizing the Bridges Marketing Toolkit!
Respite Care
A statewide Respite Care Program that offers therapeutic relief and time-limited breaks to families and other caregivers of CareSource and Aetna OhioRISE Members. Our network of respite care practitioners are available to assist members with normal activities of daily life in order to meet the needs of the primary caregiver, as well as the child.
Same-Day Access & Telehealth
With support from Aetna, the Collaborative is advancing same-day access programming for children’s behavioral health centers. Through expert consultation and development of our own proprietary toolkit, we have established improved capability to provide mental health assessments to children without appointments.
