The CIR process has been revised to account for critical incidents that may negatively impact the health and welfare of young adults or their dependents and pose a risk of harm, such as death, lifesaving medical emergencies, and unanticipated hospitalization. The categories, as noted in the attachment, are deemed critical incidents.
The CIR email notification, by noon of the first business day or within 24 hours, is sent to the required staff as noted in the attachment, with the narrative including information about the date/time the service team notified of the CI, date/time CI occurred, CIRT recommendation, CI category, and brief details about the incident absent of Private Health Information (PHI) and Personal Identifying Information (PII). This information will be copied and pasted in an activity log in SACWIS.
In lieu of using the CIR form, a more detailed narrative is noted in an Activity Living in SACWIS.
The Collaborative (Regional Coordinator and Administrative staff) and ODCY (Policy Developer) will review this information. Based on the documentation, we will determine if additional information is needed or if a CIRT is required.
The effective date is 1/1/25. We will review this new process during the December Regional Meetings.
